Applying for an Emotional Support Animal letter is straightforward. Start by filling out our online Psychological Evaluation Form, which is a questionnaire aimed at identifying and diagnosing psychological disorders according to the DSM V.
There’s nothing to download, and you can input your answers directly into the form. The Evaluation Form takes about 25 minutes to finish, so please allocate enough time to complete it. If you are unable to complete all questions leave it blank, the therapist will evaluate it and call you if needed.
After completing the form, click the “Submit” button to send it to one of our licensed professionals for review. If it is determined that you meet the criteria for a diagnosis of a DSM V disorder you will be contacted by email to schedule a follow-up telephone consultation. This telephone consultation should take less than 15 minutes and in many areas is necessary to comply with the state licensing board requirements.
